BBL With Tattoos and Stretch Marks: What Actually Happens

Tattoos stretch. Stretch marks may or may not improve. Neither disqualifies you from BBL — but knowing what actually happens prevents disappointment.

Safety standard: Every article on this site assumes fat grafting is performed subcutaneously only — never into or beneath the gluteal muscle. Intramuscular fat injection carries a fatal fat-embolism risk documented across surgical literature and is the reason for the current international consensus on subcutaneous-only technique, ideally with real-time ultrasound guidance. Colombia's accredited plastic surgeons follow this standard.

Tattoos and stretch marks are the two most-asked skin considerations before BBL — and the answers surprise many patients. Tattoos usually stretch and distort with BBL. Stretch marks can improve visually, worsen, or stay the same. Neither of these disqualifies you from BBL, but knowing what to expect prevents disappointment.

What happens to tattoos during BBL

Tattoos on donor sites (abdomen, flanks, back)

Liposuction under tattooed skin generally does not damage the tattoo image, but:

Tattoos on recipient sites (buttocks)

Tattoos in areas receiving fat grafts stretch as tissue volume increases:

Tattoo locationBBL effectPost-BBL appearance
Abdomen (lipo zone)Skin retractionMay appear more compact
Flanks (lipo zone)Skin repositioningSlight distortion possible
Lower backBoth lipo and injection zoneNotable stretching
Upper buttocksInjection zoneProportional stretching
Hip areaInjection zone (if lateral fill)Stretching depending on volume
Tattoo behavior depends on both surgical zone and image size.

The tattoo touch-up scenario

Many BBL patients plan tattoo touch-ups after their 6-month post-op point. This is typically when:

Wait at least 6 months after BBL before scheduling tattoo touch-ups or new tattoo work in treated areas.

Stretch marks and BBL — the mixed reality

Stretch marks (striae) are skin damage from rapid stretching that leaves permanent visual patterns. How they behave during BBL varies:

Existing horizontal stretch marks (from pregnancy, weight change)

New stretch marks (post-BBL)

Volume addition can cause new stretch marks in:

Most modest-volume BBLs in patients with normal skin quality don't cause new stretch marks. Very high volumes carry higher risk.

The stretch mark 'improvement' myth

Some marketing suggests BBL 'fixes' stretch marks. It doesn't. What BBL does is change the visual context — full, smooth tissue can make stretch marks appear less noticeable by proportion. But the stretch marks themselves are unchanged. If your goal is stretch mark treatment, laser treatments and microneedling address them more directly.

Stretch mark treatment options if BBL doesn't help

Fractional laser (Fraxel)

Multiple sessions of laser treatment can improve stretch mark appearance. Best for:

Microneedling with radiofrequency

Similar mechanism to Morpheus8 covered in that piece. Modest improvement over sessions.

Chemical peels

Superficial improvement for shallow stretch marks. Limited for deeper ones.

Retinoids

Topical tretinoin/retinol can gradually improve appearance over months. Cheapest option.

Timing stretch mark treatment around BBL

Recommended sequence:

  1. Complete BBL surgery
  2. Full 6-month recovery and settling
  3. Evaluate final skin appearance
  4. Begin stretch mark treatments if desired

Don't do stretch mark treatments in the 3 months pre-BBL — laser or microneedling too close to surgery can affect healing.

Skin discoloration considerations

Fitzpatrick IV-VI skin

Darker skin tones have specific considerations for both tattoos and stretch marks:

Pigmentation improvement

Skin lightening treatments (hydroquinone, tranexamic acid, azelaic acid) can address post-surgical pigmentation but require patience and consistent sun protection.

Body art considerations

Patients with significant body art often want to preserve visual composition:

What if you're planning tattoos and BBL?

If you're considering both new tattoos and BBL:

This avoids double-work and gives your tattoo artist accurate skin to work with.

Cellulite improvement (or worsening) with BBL

Cellulite (skin dimpling from fibrous tissue tethering underlying fat) can:

Dedicated cellulite treatments (Cellfina, Aveli) address the fibrous bands directly and can be combined with BBL for comprehensive improvement.

Individual variation is significant. High volumes typically produce more distortion of existing skin features.

Consult questions specific to skin

The bottom line

Existing tattoos and stretch marks don't disqualify you from BBL, but they will change with the procedure. Have realistic expectations, plan touch-ups for 6+ months post-op, and consider adjunctive skin treatments if visual improvement is your primary goal for these features specifically.
