Not every BBL produces the result the patient wanted on the first round. Revision surgery โ a second BBL procedure to improve, correct, or enhance the results of the first โ is more common than most people realise. It is not a sign that something went wrong; sometimes bodies heal differently than expected, or patients want additional volume after seeing their stabilised results. Here is what revision involves and when it makes sense.
Common Reasons for Revision
- Insufficient volume. The most common reason. If your body reabsorbed more fat than expected (the low end of the 60โ80% survival range), or if you started with limited donor fat, the final result may be smaller than you hoped. A revision BBL adds more volume.
- Asymmetry. Some degree of asymmetry is normal in every body, including after surgery. If the asymmetry is noticeable and bothersome, targeted fat injection can correct it.
- Contour irregularities. Lumps, dents, or uneven contours in the liposuction areas or buttocks can result from uneven fat removal, fibrosis, or healing variations. Revision can smooth these areas through additional liposuction or fat injection.
- Desire for more enhancement. Some patients are happy with their results but want more โ more volume, more projection, more dramatic contouring. This is an elective choice, not a correction.
- Weight loss after BBL. Significant weight loss shrinks the transferred fat along with all other body fat. A revision can restore volume, provided you have stabilised at your new weight.
- Poor initial surgery. In cases where the original surgeon was unqualified or the procedure was performed poorly, revision surgery with a skilled, board-certified surgeon can significantly improve the outcome. These cases are more complex and require a surgeon experienced in revision work.
Timing: When Can You Have a Revision?
Most surgeons recommend waiting a minimum of 6โ12 months after the original BBL before considering revision. This allows the transferred fat to fully stabilise, all swelling to resolve, and your final result to become clear. Operating on tissue that is still healing from the first procedure increases complication risk and makes it harder for the surgeon to achieve precise results.
If your reason for revision is dissatisfaction with volume, wait until month 6 at minimum. What you see at month 3 is close to final, but subtle changes continue through month 6โ12. Rushing into revision before the first procedure has fully settled can lead to over-correction.
โ ๏ธ Do not revise too early
The most common mistake with revision surgery is doing it too soon. At month 2โ3, many patients feel their result has "deflated" compared to the swollen week-2 appearance and want immediate correction. This is normal settling, not a failed result. Wait for your surgeon's recommended timeline. The fat that has survived at month 6 is permanent โ judge your results on that, not on the swelling peak.
What Revision Involves
A revision BBL is structurally similar to the original procedure: liposuction to harvest fat, processing, and re-injection into the buttocks. However, there are differences. The donor sites have already been liposuctioned once, which means less available fat and more fibrous tissue from the first surgery. The surgical technique may need to account for scar tissue in the injection areas. And expectations should be calibrated โ a revision typically adds modest enhancement rather than dramatic transformation.
Recovery follows the same protocol as the original BBL: compression garment, no sitting, lymphatic drainage, 10โ14 day minimum stay in Colombia. Most patients report that revision recovery is slightly easier than the first time because they know what to expect.
Cost
Revision BBL pricing in Colombia ranges from $2,500 to $5,500, depending on the complexity. If your revision is being performed by the same surgeon who did the original procedure, some offer discounted revision rates โ ask during your initial consultation. If you are switching to a different surgeon for revision (common when the original surgery was performed elsewhere or by an unqualified provider), expect full pricing.
Choosing a Revision Surgeon
Revision surgery is technically more demanding than a primary BBL. The donor sites have been previously liposuctioned, the buttock tissue has been previously injected, and scar tissue from the first procedure adds complexity. If your original surgeon is SCCP-certified and you are happy with their approach, they are usually the best choice for revision โ they understand what was done and can build on it.
If you are seeking revision because the original surgery was poorly performed, choose a surgeon who specifically has experience with revision cases. Ask to see before-and-after photos of revision patients, not just primary BBL patients. Revision outcomes depend on different skills than primary surgery.
โ Prevention is better than revision
The best way to avoid needing a revision is to choose the right surgeon the first time, set realistic expectations during consultation, follow all recovery protocols rigorously (garment, no sitting, lymphatic drainage, nutrition), and avoid significant weight loss in the first 6 months. None of this guarantees you will not want a revision, but it maximises the chances that your first result meets your expectations.
