They both make your buttocks bigger, but the similarity ends there. A Brazilian Butt Lift uses your own fat. Gluteal implants use silicone. The procedures, results, risks, recovery, and ideal candidates are fundamentally different. Here is how to decide.

The Core Difference

A BBL is a fat transfer procedure โ€” fat is harvested from your midsection via liposuction, purified, and injected into your buttocks. It simultaneously slims your waist and enhances your curves. The result looks and feels natural because it is your own tissue.

Gluteal implants involve placing solid silicone implants beneath or within the gluteal muscle through an incision in the intergluteal crease (between the buttocks). The result adds volume and projection but does not contour your waist โ€” that would require separate liposuction.

When a BBL Is the Better Choice

The BBL is the more popular procedure by a wide margin, particularly in Colombia where surgeon expertise in fat transfer is exceptionally deep. For most patients who have adequate donor fat, it is the recommended option.

When Implants May Be Better

Gluteal implants are a valid procedure but are less commonly performed in Colombia's medical tourism market. Fewer surgeons specialise in implants compared to BBL, and the complication profile is different โ€” implant displacement, capsular contracture (hardening of scar tissue around the implant), and potential need for future replacement are specific to implant surgery.

โœ“ What most surgeons will tell you

If you have enough fat for a BBL, the BBL is almost always the better option. It provides dual contouring (waist + buttocks), uses your own tissue, looks and feels more natural, has no risk of implant-related complications, and does not require future replacement. Implants serve a real need for patients who lack donor fat, but they are the exception rather than the rule.

Can You Combine Both?

Yes โ€” a "hybrid" approach uses implants for base projection and fat transfer on top for natural shape and contouring. This is uncommon and significantly more complex. It is typically reserved for patients who want maximum augmentation beyond what fat transfer alone can achieve. If a surgeon recommends this approach, make sure they have specific experience with the combination.
